Anemia is one of the most frequent clinical presentations worldwide, defined by a decrease in red blood cells (RBCs) or hemoglobin, resulting in diminished oxygen delivery to peripheral tissues. The most practical clinical approach classifies anemia based on Mean Corpuscular Volume (MCV):

🔍 Morphological Classification (MCV-Based):

* Microcytic Anemia (Low MCV < 80 fL):

* Common Causes: Iron Deficiency Anemia (IDA), Thalassemia, Sideroblastic Anemia, Lead poisoning.

* Key Mechanism: Defective hemoglobin or heme synthesis, leading to small, pale cells (microcytic, hypochromic).

* Macrocytic Anemia (High MCV > 100 fL):

* Common Causes: Vitamin B12 deficiency (pernicious anemia), Folate deficiency, liver disease, alcohol use.

* Key Mechanism: Impaired DNA synthesis resulting in megaloblastic maturation arrest and abnormally enlarged erythrocytes.

* Normocytic Anemia (Normal MCV 80–100 fL):

* Common Causes: Anemia of Chronic Disease (ACD), Acute Blood Loss, Early Aplastic Anemia, Hemolytic conditions.

* Key Mechanism: Decreased production, acute loss, or accelerated peripheral destruction of structurally normal RBCs.

⚠️️ Key Physical Clues & Exam Findings:

* Pallor: Skin & conjunctival pallor reflecting low hemoglobin concentration.

* Systemic Fatigue & Lethargy: Hypoperfusion and reduced tissue oxygenation.

* Glossitis: Smooth, painful, erythematous tongue (classic in nutritional B12/iron deficiencies).

* Koilonychia: Spoon-shaped nails frequently seen in advanced iron deficiency.

🧪 Essential Workup & Monitoring:

* Complete Blood Count (CBC) with RBC indices

* Reticulocyte count (marrow response check)

* Serum Ferritin, Iron, TIBC & Transferrin saturation

* Serum Vitamin B12 & RBC Folate levels

* Peripheral Blood Smear (PBS) morphology
